Shall Fire Protection District No. 4 of the Parish of Beauregard, State of Louisiana (the "District"), levy a special tax of twenty-four and fifty-two hundredths (24.52) mills on all the property subject to taxation in the District (an estimated $393,000 expected at this time to be collected from the levy of the tax for an entire year), for a period of fifteen (15) years, beginning with the year 2018 and ending with the year 2032, for the purpose of acquiring, constructing, improving, maintaining and/or operating fire protection facilities and equipment in and for the District, and paying the cost of obtaining water for fire protection purposes, said millage to represent a one and fifty-two hundredths mills (1.52) increase over the 23 mills tax authorized to be levied through the year 2017 pursuant to an election held on January 18, 2003?

Shall the Calcasieu Parish Police Jury, serving as the governing authority of Calcasieu Parish, Louisiana, the ("Parish") be authorized to renew, levy and collect an existing special tax of three and forty-nine hundredths (3.49) mills, on all property subject to taxation in the Parish, for a renewed period of ten (10) years, beginning January 1, 2014 and ending December 31, 2023, with collections from the renewed levy estimated to be $5,764,353 for one entire year, for the purpose of maintaining and operating the Juvenile Detention Center and Juvenile Probation Office; paying the cost of improvement to or construction of facilities at the Juvenile Detention Center, constituting works of permanent public improvement, title to which shall be in the public; and providing and/or contracting for the provision of juvenile care, shelter, evaluations, treatment and counseling services, with the proceeds of the renewed tax, to be used to the extent and in the manner permitted by the laws of Louisiana, including, but not limited to, Part III, Chapter 4, Title 39 of the Louisiana Revised Statutes of 1950, as amended?

PW PJ (Road and Drainage) – 4.06 mills, PJ 10 years

Shall the Calcasieu Parish Police Jury, serving as the governing authority of Calcasieu Parish, Louisiana, (the "Parish") be authorized to renew, levy and collect an existing special tax of four and six-hundredths (4.06) mills, on all property subject to taxation in the Parish for a renewed period of ten (10) years, beginning January 1, 2014 and ending December 31, 2023, with collections from the renewed levy estimated to be $6,705,809 for one entire year, for the purposes of paying the cost of expenses, equipment and supplies related to construction, hardsurfacing and improving roads, highways, and bridges, and the necessary ditching and drainage related thereto; continuation of a road and drainage trust fund in the amount of $1,000,000 annually to be appropriated to projects which have local, state, or federal matching funds; and in the event the proposition carries, for authority to levy the renewed special tax provided for herein?

PW PJ (Parish Health Unit) – 2.48 mills, PJ 10 years

Shall the Calcasieu Parish Police Jury, serving as the governing authority of Calcasieu Parish, Louisiana, (the "Parish") be authorized to renew, levy and collect an existing special tax of two and forty-eight hundredths (2.48) mills, on all property subject to taxation in the Parish for a renewed period of ten (10) years, beginning January 1, 2014 and ending December 31, 2023, with collections from the renewed levy estimated to be $4,096,159 for one entire year, for the purpose of maintaining and operating the facilities and services of the Calcasieu Parish Health Unit; paying the cost of improvement to or construction of health facilities of the Parish, constituting works of permanent public improvement, title to which shall be in the public; and providing and/or contracting for health-related services and responsibilities under the authority of the Police Jury, with the proceeds of the renewed tax to be used to the extent and in the manner permitted by the laws of Louisiana, including, but not limited to, Part III, Chapter 4, Title 39 of the Louisiana Revised Statutes of 1950, as amended?

School District No. 25, $13,200,000 bonds – SB 20 years

Shall School District No. 25 of Calcasieu Parish, Louisiana, incur debt and issue bonds in an amount not exceeding $13,200,000 for a period not to exceed twenty (20) years from the date thereof, with interest at a rate not exceeding eight (8%) percent per annum, for the purpose of acquiring and/or improving lands for building sites and playgrounds, purchasing, erecting, enlarging and/or improving school buildings and other school related facilities within and for said School District, and acquiring the necessary equipment and furnishings therefor, title to which shall be in the public, which said bonds shall be retired with, paid from and secured by ad valorem taxes on all taxable property within the limits of School District No. 25 of Calcasieu Parish, Louisiana, sufficient in rate and amount to pay said bonds in principal and interest, with the estimated millage rate to be 26.05 mills in the first year of issue?

School District No. 26, $10,000,000 – SB 20 years

Shall School District No. 26 of Calcasieu Parish, Louisiana, incur debt and issue bonds in an amount not exceeding $10,000,000 for a period not to exceed twenty (20) years from the date thereof, with interest at a rate not exceeding eight (8%) percent per annum, for the purpose of acquiring and/or improving lands for building sites and playgrounds, purchasing, erecting, enlarging and/or improving school buildings and other school related facilities within and for said School District, and acquiring the necessary equipment and furnishings therefor, title to which shall be in the public, which said bonds shall be retired with, paid from and secured by ad valorem taxes on all taxable property within the limits of School District No. 26 of Calcasieu Parish, Louisiana, sufficient in rate and amount to pay said bonds in principal and interest, with the estimated millage rate to be 14.14 mills in the first year of issue?

School District No. 30, proposition 1 -- $4.6 million bonds, SB 20 years

Shall School District No. 30 of Calcasieu Parish, Louisiana, incur debt and issue bonds in an amount not exceeding $4,600,000 for a period not to exceed twenty (20) years from the date thereof, with interest at a rate not exceeding eight (8%) percent per annum, for the purpose of acquiring and/or improving lands for building sites and playgrounds, purchasing, erecting, enlarging and/or improving school buildings and other school related facilities within and for said School District, and acquiring the necessary equipment and furnishings therefor, title to which shall be in the public, which said bonds shall be retired with, paid from and secured by ad valorem taxes on all taxable property within the limits of School District No. 30 of Calcasieu Parish, Louisiana, sufficient in rate and amount to pay said bonds in principal and interest, with the estimated millage rate to be 1.07 mills in the first year of issue?

School District No. 30, proposition 2, $5.2 million bonds, SB 20 years

Shall School District No. 30 of Calcasieu Parish, Louisiana (the "District") incur debt and issue bonds in an amount not exceeding $5,200,000 for a period not to exceed twenty (20) years from the date thereof, with interest at a rate not exceeding eight (8%) percent per annum, said bonds authorized for the purpose of renovating and improving the football stadium and related athletic facilities at Sulphur High School and acquiring the necessary equipment and furnishings therefor, and for acquiring and/or improving lands for building sites and playgrounds, purchasing, erecting, enlarging and/or improving school buildings and other school related facilities within and for the District, title to which shall be in the public, which bonds shall be retired with, paid from and secured by ad valorem taxes on all taxable property within the limits of the District sufficient in rate and amount to pay said bonds in principal and interest, with the estimated millage rate to be 1.22 mills in the first year of issue?

City of Sulphur, Proposition 1 – 5 mills (fire department) CC 10 years

Shall the City of Sulphur, Louisiana, levy and collect each year for a period of ten (10) years on all property subject to taxation within the corporate limits of the City of Sulphur, Louisiana, a special tax of five (5) mills on the dollar, beginning with and including the year 2015, with collections from the levy of the tax estimated to be $587,221 for one entire year, for the purpose of purchasing additional equipment for, and enlarging and improving the present facilities of, the Fire Department of the City of Sulphur, Louisiana, for the maintenance of said equipment, improvements and facilities, and for salaries for the maintenance and operation of said department, constituting works of permanent public improvement, title to which shall be in the public?

City of Sulphur, Proposition 2 – 5 mills (Streets) CC 10 years

Shall the City of Sulphur, Louisiana, levy and collect each year for a period of ten (10) years on all property subject to taxation within the corporate limits of the City of Sulphur, Louisiana, a special tax of five (5) mills on the dollar, beginning with and including the year 2015, with collections from the levy of the tax estimated to be $587,221 for one entire year, for the purpose of graveling, shelling, hard-surfacing, installation of culverts and drains, and otherwise improving the streets of the City of Sulphur, and for the maintenance thereof, constituting works of permanent public improvement, title to which shall be in the public?

City of Lake Charles, Develop, finance hotel and facilities – CC

Shall the City of Lake Charles, Louisiana, pursuant to Louisiana Revised Statutes 33:4699.1 and 33:9038.57, et seq., (collectively the "Act") and other statutory authority, be permitted to develop and finance a hotel and other facilities, described in the Act, within the area described in the Act (the "Development Area"), and to pay all or a portion of the costs of projects associated with infrastructure improvements to be paid from and secured by the pledge and dedication of sales and use taxes levied and collected within the Development Area, as permitted in the Act, through issuance of bonds, sales tax increment financing or other financings, all for economic development purposes?
